Job Summary: We are seeking a compassionate and dedicated Social Worker who desires to make difference in someone's lives to join our team, a not-for-profit organization. The Social Worker will be responsible for providing support, advocacy, and resources to residents and their families to ensure their overall well-being to meet the organization's mission, vision and values and in accordance with company policies and procedures and rules and regulations. Pay Range: $30.15 to $38.81 per hour Medical Insurance (including Drug, Vision, and Dental coverage) 401K (We offer a 4% matching contribution, with Traditional and Roth 401k options) 10 paid holidays per year 10 days of paid vacation, with increases at 4yrs and 7yrs of service 10 days of paid sick leave Employee Referral Bonus Discounted Employee Meals Free On-Site and Street Parking Evening and Night Differential pay for direct care positions Nurse Aide Tuition Reimbursement program Flexible Spending Account (tax saving benefit for eligible expenses) AFLAC Supplemental Insurance Responsibilities: Conduct assessments to identify residents' psychosocial needs. Develop care plans in collaboration with residents, families, and healthcare providers. Provide counseling and support to residents and families to address emotional and social-related issues. Advocate for residents' rights and coordinate with external agencies as needed. Maintain accurate and up-to-date documentation of services provided. Participate in care conferences and team meetings to provide input on residents' care plans. Collaborate with other healthcare professionals to ensure holistic care for residents. Investigate grievances and alleged abuses. Assist residents with admission and discharge planning. Participates in Quality Assurance Program to achieve and maintain a 5 Star Rating facility. Qualifications: Bachelor's or Master's degree in Social Work or healthcare related field. Previous experience working in a nursing home or long-term care setting is preferred. Strong communication and interpersonal skills. Ability to work collaboratively with a multidisciplinary team. Knowledge of social service resources and agencies.
